Job Description

The Courville at Nashua is a Senior Healthcare Community in Nashua, NH. We are locally owned and operated and committed to providing personalized service and care. We are looking for a part-time Admissions Assistant :

  • M-F schedule
    • 24 hrs/wk
    • Flexible hours

  • Assist the Admissions Director as needed
  • Complete admissions agreements
  • Support the admissions process
    • Facilitate a smooth and welcoming experience
    • Have a personal conversation with each patient/resident
    • Follow-up with family
    • Anticipate needs and next steps
    • Coordinate with care team - nursing, food services, activities, etc.
  • Collect necessary documents: social security card, insurance card, etc.
    • Upload clinical and finacial doucmentation into PCC once a bed is accepted 
    • Verify all contact information in PCC is accurant and complete
    • Complete Sign-Ins within 48-72 hours of eah admission
    • Maintain accurate documentation for state review and auditing purposes.
  • Confirm admission documentation from the previous day has been uploaded into PCC
  • Collect payment for private pay and respite admissions
  • Support the discharge process using the 30-day discharge tracker
  • Review rooms to ensure empty rooms/beds are cleaned, made, and ready for new admissions and tours
  • Assist social services as needed with discharge documentaiton and scheduling meetings

Qualifications

  • No prior SNF experience required .
  • Experience with  PCC (PointClickCare)or similar EMR systems is highly preferred.
  • Excellent verbal and written communication skills; ability to communicate professionally with families, referral sources, and internal teams.
  • Strong organizational and time-management skills.
  • High attention to detail and accuracy in documentation.
  • Ability to work collaboratively with clinical and administrative leadership.
  • Compassionate, resident-focused approach with the ability to support families during transitions.
