Hospice On-Call Registered Nurse - Full Time & Part Time

We're currently seeking a passionate and skilled Registered Nurse (RN) to join our ever-expanding Hospice team in the Pottsville, PA and surrounding areas. This role offers the chance to directly provide patient-centered care in the home that focuses on comfort, dignity, and support during life's most challenging moments, truly making a difference in the lives of our patients.

What You'll Do:

• Conducting Start of Care comprehensive assessments.
• Drafting care plans, reconciling patient medications, and ensuring orders for care are accurate and complete.
• Preparing accurate, orderly, and timely documentation.
• Coordinating care with patient and provide proper reports to Case Manager.
• Provides health care instructions to the patient/family as appropriate per assessment and plan.

Qualifications

About You:

• An active RN license in the state in which practicing (Minimum of 1 year of experience preferred).
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Compassionate and empathetic approach to patient care.
• Ability to work independently and as part of a team.
• Knowledge of hospice regulations and standards of care.
• Reliable transportation and willingness to travel.
